Intake Materials for SMC:
This download (see link below) is sort of like the express line in the supermarket. We can do our own job better if we have all of your relevant information at the time of the intake, including medical history, information about accidents or traumas (dates, nature of injury) previous treatments tried, or any information you think we should have. The “LENShandout” is an informational document to help prospective clients understand the LENS process.
